Palliative care focuses on providing patients relief from pain and other symptoms of a serious
illness, no matter the diagnosis or stage of a disease. Palliative care teams aim to improve your quality of life during treatment. Our team of doctors, nurses and other specially trained professionals will work with you, your family, and your doctors to complement your ongoing care and improve your quality of life.
During the consultation Your palliative care team will discuss your symptoms, current treatments,
and how your illness is affecting you and your family. Together, your palliative care team
will make a plan to prevent and ease suffering, which will improve your quality of life. They will
collaborate with a team of social workers, psychologists, and nutritionists, as well as your doctors to ensure your care is well-coordinated. The goal of the palliative care team is to discuss what’s most important to the patient so you can live your best life possible.
After the consultation Your palliative care plan is designed to fit your life and your needs. It may include elements such as:
Symptom management: Your palliative care plan will include steps to address your symptoms and improve your comfort and well-being. This includes easing the physical, psychological, and social problems related to your illness. Your care team will be glad to answer any questions you may have.
Support and advice: Your palliative care team will take the time to listen to your concerns and desires. They will discuss your goals of care, educate you about your diagnosis, and address any questions or concerns you may have. By helping patients understand their condition and treatment, the palliative care team helps patients navigate difficult situations and assists with making important decisions.
Care techniques: help ease your mind while you focus on healing. These include breathing techniques, visualization, or simply listening to music. Our experts are specially trained in tailoring each patient’s therapy to their unique needs.
Advance care planning: A palliative care team member can talk with you about goals and wishes for your care to ensure they are met with the highest standards. This information could then be used to help you develop a living will, advance directive and a health care power of attorney.
Meet Our Physicians
Isaac Hardoon, DO is dual boarded in Internal Medicine and Hospice and Palliative Medicine. Dr. Hardoon is a graduate of Stony Brook University where he received a Bachelor of Arts in Theater Arts with a Minor in Biology. He obtained his Doctor of Osteopathy from West Virginia School of Osteopathic Medicine and completed his internship and residency in the Department of Medicine at Maimonides Medical Center in Brooklyn. Janaya Raynor, MD, MS obtained a Master of Science degree at Johns Hopkins University. Continuing her education, she received a Doctor of Medicine from Ross University. She completed her Internal Medicine Residency and Fellowship in Hospice and Palliative Medicine at Jamaica Hospital Medical Center. MaryAnn Fragola, DNP, ANPc, ACHPN is a board-certified oncology nurse practitioner. She has been a part of this practice for the past 20 years providing direct patient care to patients and families.
