• Insurance information and referring provider contact details
• Reason for referral and urgency level
• Most recent office note and relevant history
• Pathology reports (if available)
• Recent labs (CBC, CMP, relevant tumor markers if applicable)
• Imaging reports and images (CT, MRI, PET, US, mammography, etc.)
• Medication list and allergies
• Prior treatment history (chemo, radiation, surgery) when applicable
